Altough you can’t see the persons face, i think that the close up on what is going on in the picture is depressing in it self. Alot of people link depression with self harming as this image is displaying. Altough there is not a huge amount of suspense in this picture there is still an element of anticipation that is portrayed through the actions. By adding dramatic music and enhancing the lighting it would make make this scence look more suspenseful.

Mystery
Opened creepy doors are typically associated with scences in films that usually create suspense. In this picture you can just see the door open with no one in the shot which immediately creates mystery. Mysteries are used to build up anxiety and create restlessness which i think this image does.
